Legislative & Policy Outlook Panel

The Legislative and Policy Outlook Panel brings together national leaders from across EMS and Mobile Integrated Health/Community Paramedicine (MIH/CP), including the National Association of Mobile Integrated Healthcare Providers (NAMIHP), the American Ambulance Association (AAA), and leaders with a State EMS Director lens for a timely discussion on the future of the field. Panelists will share updates on federal and state legislative activity, regulatory priorities, and payer policy developments shaping the EMS and MIH/CP landscape.

The session will explore key challenges in the year ahead, including funding sustainability, workforce issues, and the evolving role of EMS and MIH in value-based care. Special attention will be given to rural health transformation efforts, highlighting opportunities where MIH/CP can address access gaps and support innovative care delivery models. Looking ahead to 2026 and beyond, panelists will consider how MIH/CP can align with national healthcare priorities, strengthen advocacy strategies, and ensure a strong policy foundation for continued growth and impact.

Where to Begin? A Fire Department’s Approach to Launching an MIH Program

This presentation outlines the development and implementation of the Mobile Integrated Healthcare (MIH) program in Chesapeake, Virginia, led by a single program manager with no previous experience in MIH-CP programs. The program aimed to fill gaps in healthcare access and decrease non-emergency 911 calls through paramedicine and community services. A community needs assessment, including call volume analysis and emergency department data, guided the program’s strategy, particularly in light of an expected increase in the elderly population. Unlike traditional community paramedicine, Chesapeake’s approach focused on patient navigation, integrating healthcare and social services, and providing education for chronic disease management. Core services included fall prevention, chronic illness support, and patient education. The program’s success relied on thorough planning, data collection for evaluation and funding, stakeholder engagement, and effective marketing. Pilot results demonstrated significant reductions in 911 calls among enrolled clients, and strategic partnerships with local organizations improved service delivery and community impact.

This presentation highlights how fire-based Mobile Integrated Healthcare (MIH) programs can transform chronic disease management and support aging in place by integrating with Federally Qualified Health Centers (FQHCs). Leveraging multidisciplinary teams and innovative partnerships, the Olathe Fire Department MIH program demonstrates how aligning emergency medical services with primary care and community resources reduces 911 utilization, prevents unnecessary hospital admissions, and improves continuity of care. Case examples will illustrate how MIH providers address chronic care needs, support geriatric populations, and build collaborative treatment plans that account for both medical conditions and social determinants of health. By focusing on the whole patient, including their healthcare access, home environment, and long-term goals, fire-based MIH programs become a vital component of community health and safety.

Measuring What Matters: The Evolution of Data-Driven Community Paramedicine

The Eagle County Paramedic Services’ Community Paramedicine program emerged over a decade ago as one of the first programs of its kind in Colorado and the United States. The evolution of this program has led to its becoming an integral piece of the local healthcare system in Eagle County, breaking barriers for the underserved and saving millions in healthcare costs. This success has been in large part due to a continued journey towards creating a fully integrated health system that includes a multidisciplinary and interagency approach to care.
 
In this presentation, we present clinical data that illustrates the impact of our services on individual health outcomes, along with a description of data-driven quality improvement processes that have shaped our program goals and align with broader healthcare system quality measures. We outline how we continue to leverage different health information technology systems to expand integration with our local healthcare partners.

Historically focused on ambulance transport, traditional payor models currently incentivize unnecessary ED visits, resulting in sub-optimal care for patients and a misuse of resources for EMS.
